Religious Formation Ministry

Download the “Religious Formation Enrollment Forms 2009–2010” packet of five pages — the cover letter that explains Church of the Ascension’s policies for religious formation and sacrament preparation; Student Information Form (one per child registering); General Consent Form (one per child registering); and Sacramental Costs 2009–2010 form.

Completely fill out all forms including marking appropriate spaces to calculate your family’s costs.

If you have an older child or other family member desiring to receive the sacrament of Confirmation, please click on “Directions for Confirmation Preparation Enrollment” on the main Religious Formation Ministry web page.

If your child is in the 3rd Grade, she/he will receive both the sacrament of Confirmation and the Sacrament of First Eucharist on Sunday, April 25th, at the 5:00pm mass. Please download the “Confirmation/First Eucharist Enrollment Form” on the main Religious Formation Ministry web page in addition to the above forms.

If your child is in the 2nd grade, she/he will be prepared to receive the Sacrament of Reconciliation on February 23, 2010. Be sure to mark “First Reconciliation” on the “Sacramental Costs 2009–2010” form.

Completed forms may be dropped off at the parish office, 8:30 am to 5:00 pm, daily, or on Sunday morning, 7:30 am to 11:30 am.
