Religious Formation Ministry

Welcome to Church of the Ascension’s Confirmation preparation pages. Please follow the directions below to read and download the information and forms needed to begin preparation for receiving the sacrament of Confirmation on Sunday, April 25th, at the 5:00pm mass. If you have any questions, please feel free to call the Religious Formation Office at the parish, 480 837–1066 x26.

Download one “Confirmation Enrollment Form” on the main Religious Formation Ministry web page for each child or adult desiring Confirmation and fill out the form.

Download the “Sacramental Costs 2009–2010” or “Adult Confirmation Cost Sheet” on the main Religious Formation Ministry web page in order to calculate cost for these family members. There is no fee for the sacrament but we still need to ask for payment to cover the costs of all materials and supplies used in the preparation process.

Middle School and High School Teens must be active in Church of the Ascension’s Youth Groups. Click on “Faith Formation”, “Middle School Youth Ministry” or “High School Youth Ministry”, and “Youth Group Enrollment” to download enrollment material.

Elementary school children need to be active in Church of the Ascension’s Religious Formation Program. Click on “Faith Formation”, “Religious Formation (k–5th grade)”, and “Religious Formation Enrollment Forms 2009–2010” to download enrollment material.

Completed forms may be dropped off at the parish office, 8:30 am – 5:00 pm daily, or on Sunday morning 7:30 am – 11:30 am.
